Request a PATCH Plan

Providers can request a PATCH plan through Epic for inpatients or outpatients. Outside providers can request a PATCH Child Life Referral via Physician’s Connect or our online form to help create a tailored Adaptive Care Plan for the patient.

The Patient Assistance Team at Children’s Nebraska (PATCH) is a program that helps us improve the hospital experience for children with autism spectrum disorder or other developmental, neurological and behavioral challenges.

How to Request a PATCH Plan for Your Patient

Children’s providers can request a PATCH plan for a patient through EpicCare Link by placing a PATCH Child Life Consult Order for inpatients or initiating a PATCH Child Life Referral for outpatients. For providers outside the Children’s network scheduling a patient with autism for services at a Children’s facility, you can also request a PATCH Child Life Referral via EpicCare Link or email us directly. We’ll collaborate with the family to create an Adaptive Care Plan for their upcoming visit.
