Physical Exams

Preparing for the school year includes ensuring your child meets the states health requirements with necessary physical exams, vision screenings and sports physicals.

Back-to-School Physicals

Kindergarteners and 7th graders are required to have a physical examination by a qualified physician within six months before the first day of school. If you do not wish for your child to have a physical, a waiver form is available to sign upon request. Back-to-school physicals ensure your child is progressing physically, emotionally and developmentally while meeting all school enrollment requirements.

Nebraska state law requires a vision screening for all students entering kindergarten and all out-of-state transfer students. Incoming 7th-graders are highly encouraged to have a vision screening, but it is not required. Kindergarten and 7th-grade students are also encouraged to have a hearing screening, but it is not required.

Sports Physicals

Nebraska state law requires sports physicals to be completed after May 1 and turned in before the first day of your child’s sports practice. Students may not participate in sports practices or games until the physical form is completed and returned to school. No waivers are allowed for sports physicals.

Some schools allow 7th graders to use their 7th-grade physicals as their sports physicals. Check with your school.
